Who can use this form
Organizations with a Ferment-on-premises Licence need to use the form. You need to submit the report by March 1 each year.
What you need to do
- Complete the form.
- Send your completed form by mail, fax or email. Or visit the Alcohol, Gaming, Fuel and Tobacco Division.
Cost
There is no cost to submit the form.
Before you start
Make sure you have:
- Ferment-on-premises Licence number
- total volume of beer produced per calendar year
- total volume of wine produced per calendar year
