Use the Firewood Declaration Form if you grow and cut your own wood to heat your home. You only need to submit the form as part of the Heating Assistance Rebate Program application process if the program contacts you to complete it.
Who can use this form
You need to submit the Firewood Declaration Form if:
- you grow and cut firewood on your property
- the firewood you cut is used to heat your home at the address you list on your Heating Assistance Rebate Program application
- the address you list in your Heating Assistance Rebate Program application is your primary residence
