Tenant's Notice to Quit: Early Termination of Tenancy (Form G and Form H)
Tenants use Form G and Form H to end their tenancy (lease) early if their health has gotten worse or they’ve been accepted into a nursing home.

Form G and Form H includes Form G: Tenant’s Notice to Quit and Form H: Physician’s Statement.
More than 1 tenant in the unit
If there’s more than 1 tenant in the unit, the tenancy (lease) ends for all tenants after providing Form G: Tenant’s Notice to Quit to the landlord. Other tenants can sign a new lease with the landlord if the landlord agrees.
Give notice
By law, you must give your landlord at least 1 month’s notice to end the tenancy (lease) early for health reasons or because you got accepted into a nursing home. You need to give notice before the day of the month that rent is due.
You can also provide a forwarding address for the landlord to return your security deposit.
Who can use this form
A tenant can use this form to give their landlord notice that they’re ending the tenancy (lease) early for the following reasons:
- the tenant’s health has gotten worse and they can no longer afford to pay the rent
- the tenant’s health has gotten worse and they can no longer stay in the unit or the unit is no longer accessible to them
- the tenant has been accepted into a nursing home or home for special care
What you need to do
- Complete Form G.
- If you’ve experienced a significant deterioration in health, get your doctor to complete Form H: Physician’s Certificate – Termination of Tenancy for Health Reasons.
- If you’ve been accepted into a nursing home, provide the acceptance letter from the nursing home.
- If you have co-tenants that agree with ending the tenancy (lease) early, you also need to complete Form G: Acknowledgement of Service.
- If you have co-tenants that don’t agree with ending the tenancy (lease) early, you need to serve the other tenants with a copy of Form G. Then you need to complete the Affidavit of Service (on Form G).
- Keep a copy of the forms for your records.
- Formally serve (deliver) the completed Form G and either Form H or the acceptance letter from the nursing home to your landlord. Check the form for details on delivery options and instructions on how to formally serve the landlord.
