If you’re applying for a rebate through the Property Tax Rebate for Seniors, you need to submit the Municipal Property Tax Sheet if you don’t have your property tax bill and receipts showing payment of your 2018 municipal residential property taxes. You need to have your municipality complete the form.

Use the Municipal Property Tax Sheet if you don’t have your property tax bill and receipts showing payment of your 2018 municipal residential property taxes. You need to submit the form as part of the Property Tax Rebate for Seniors application process.

Municipality availability

You need to have your municipality complete the form for you. Your municipality should send the completed form to you by mail. Or you can ask the municipality if you can pick up the form when it’s ready.

Your municipality may take 4 to 6 weeks to return the completed form to you. If you’re applying for the Property Tax Rebate for Seniors close to the 31 December 2019 deadline, make sure that your municipality is available to complete and return the form to you before the deadline. Or have the municipality complete the form well in advance of the deadline.

Who can use this form

You need to submit the Municipal Property Tax Sheet if:

  • you don’t have a detailed breakdown of your 2018 municipal residential property taxes
  • you don’t have a receipt showing that you paid your 2018 municipal residential property taxes in full
