Bingo financial report: Charitable Bingo Lottery Licence

Who can use this form

Charitable Bingo Lottery Licence holders that run a series of bingo games with total yearly prizes over $30,000 or a single bingo game with total prizes over $2,500 need to use this form.

For a series of bingo games, the report is due monthly. You need to submit the report within 30 days of the end of the month.

For a single bingo game, you need to submit the report within 30 days of the end of the month that the game occurred in.

What you need to do

1. Complete the form.
2. Include monthly fee payment with your form if you run a series of bingo games.
3. Within 30 days of the end of the month, send your completed form and payment by mail, fax or email. Or visit the Alcohol, Gaming, Fuel and Tobacco Division.

Cost

There is no cost to submit the report, but you need to pay a fee when you submit the report. The fee is 2.13% of the total value of prizes awarded in the month you’re reporting on for a series of bingo events with a total yearly prize amount of over $30,000 or for the game you’re reporting on for a single bingo game. You need to submit the fee with your Bingo Financial Report each month.

Payment options

Visa, MasterCard, debit card, cheque, money order, cash. A cheque or money order should be made payable to the Minister of Finance. Payment options may vary depending on how you submit the form.

• 2.13% fee based on total value of prizes awarded

• amount deposited into bingo account
