Who can use this form

The person named on a Birth Certificate (certificate holder) or a parent whose name appears on the child’s Birth Registration needs to use this form to authorize an agent or representative (third party) to apply for a Birth Certificate on their behalf. The agent or representative (third party) also needs to complete part of the form.

You don’t need to complete the form for an authorized legal representative who meets 1 of the following criteria:

  • they’re a lawyer working on behalf of the certificate holder
  • they’re a lawyer working on behalf of the parents of the certificate holder
  • they’re an executor or trustee of an estate
  • they have legal guardianship of the certificate holder

What you need to do

  1. Complete the form.
  2. Both parties sign the form in the presence of a person authorized to take sworn statements, like a lawyer, justice of the peace, notary public or commissioner of oaths.
  3. Agent or representative submits the form when they apply for a Birth Certificate.

Cost

There is no cost to submit the form, but you need to pay a fee when you apply for a Birth Certificate.
