Businesses, organizations and individuals that are applying for a Permanent Liquor Licence or establishments with a Permanent Liquor Licence that are applying to have a new manager or principal person approved need to use this form to have the Alcohol, Gaming, Fuel and Tobacco Division complete their criminal record check.
