Change your address: Health Card
You need to notify the Health Card Registry if your address changes.
Use the Change an Address service to update your address. You can also use this service to change your address with Registry of Motor Vehicles.
You need to provide your new address to the Health Card Registry when you move. If your Health Card has expired, the Health Card Registry processes your address change and sends you the Health Card Renewal Form.
Your address doesn’t display on your Health Card, so you don’t need to replace your existing Health Card when your address changes.
Registry of Motor Vehicles
When you update the address for your Health Card, you can also change your address with Registry of Motor Vehicles.
Who can use this form
You need to use this form to change the address for your Health Card with the Health Card Registry.
What you need to do
- Complete the form online.
- Submit your completed form.
- The service emails you a confirmation. Keep a copy for your records.
- The Health Card Registry updates your records.
How long it takes
It should take at least 1 month to update your address. It can take longer if more information is needed or if your form hasn’t been filled in correctly.
Cost
There is no cost to change your address.
Before you start
Make sure you have:
- Health Card number
- address on file with the Health Card Registry
Online
You can’t access the form online if you’re outside Canada or your computer’s IP address is outside Canada.
