File a Supreme Court appeal of a public sector body’s decision to disclose your personal or business information in response to a Freedom of Information and Protection of Privacy (FOIPOP) report by the Information and Privacy Commissioner.
Use Form 11: Appeal if you’re unsatisfied with a public sector body’s decision to disclose your personal or business information in response to a Freedom of Information and Protection of Privacy (FOIPOP) report by the commissioner.

Make sure the commissioner has already issued a report and the public sector body has made a decision in response to that report (or failed to make a decision within 30 days).

Who can use this form
You need to use this form if a public sector body has notified you that your information is being disclosed in response to the commissioner’s review of a Freedom of Information and Protection of Privacy decision.
You have 30 days to file your appeal of the public sector body’s response to the commissioner’s report.
What you need to do
- Complete the form, except for Part B: Notice of Appeal.
- Confirm that there’s not a third party or if there is a third party, include a copy of the third party’s written consent.
- Provide all required supporting documents.
- Mail the form and supporting documents to the Prothonotary of the Supreme Court of Nova Scotia.
