Request to correct your personal information under FOIPOP (Form 2)
You can submit a formal request to have a government department or public sector body correct your personal information under the Freedom of Information and Protection of Privacy (FOIPOP) Act.
Use Form 2: Request for Correction of Personal Information to formally request that a government department or public sector body corrects any errors in personal information it holds about you. Or use the form as an example and prepare your own documents. If you prepare your own, you need to include all details outlined on the form.
Error correction
If the error is minor, like a spelling mistake on a Driver’s Licence, you can first try to get the error fixed by contacting the appropriate government department or public sector body directly.
If the government department or public sector body makes the correction, it has to notify any other departments, public sector bodies or third parties it has shared that information with about the correction. The other departments, public sector bodies and third parties also have to make the correction.
If the department or public sector body decides not to make the correction you asked for, it has to make a note indicating what correction you asked for.

Who can use this form
You can use this form if you want to formally request that a government department or public sector body corrects personal information it holds about you. You can use this form with any government department or public sector body within the province.
What you need to do
- Complete the form or prepare your own documents.
- Send your completed form or prepared documents by mail or email.
How long it takes
It should take 1 to 2 weeks to hear back that your request has been received. The department or public sector body you hear back from will let you know how long your request should take to complete. It can take longer if more information is needed or if your form hasn’t been filled in correctly.
Cost
There is no cost to submit the form.
