Provide feedback about the Employee and Family Assistance Program

You can provide feedback about the service you or a family member receives through the Employee and Family Assistance Program (EFAP).

Use the Employee and Family Assistance Program Feedback Form to provide feedback or make a complaint. The Employee and Family Assistance Program (EFAP) is provided by LifeWorks.

Direct feedback

At any point in your Employee and Family Assistance Program counselling experience, you can bring forward issues, ask questions or provide feedback directly to LifeWorks by calling them at 1-800-777-5888. LifeWorks keeps all information you provide to them confidential.

Feedback through the Public Service Commission

If you would like the Public Service Commission to investigate or ask question on your behalf, complete the Employee and Family Assistance Program Feedback Form.

The form provides the Public Service Commission with your permission to discuss your question or concern with a representative of LifeWorks. It does not allow us to discuss any of the content of your counselling sessions.

Who can use this form

Government of Nova Scotia employees need to use this form if they want the Public Service Commission to investigate or ask a question to LifeWorks on their behalf.

What you need to do

  1. Complete the form.
  2. Check the form for details on all required supporting documents.
  3. Send your completed form and supporting documents by mail or email.
  4. You get a response from the Benefits Unit if you requested a response.

How long it takes

It should take 3 to 5 days to get a response from the Benefits Unit if you requested a response. It can take longer if more information is needed or if your form hasn’t been filled in correctly.

Cost

There is no cost to submit the form.

  • someone available to witness your signature
