Consent to disclose and use personal Information (Form 4): FOIPOP requests

Give written permission for government departments or public sector bodies to disclose and use your personal information as requested under the Freedom of Information and Protection of Privacy (FOIPOP) Act. Public sector bodies can use program-specific consent forms if they choose.

Or use the form as an example and prepare your own documents. If you prepare your own, you need to include all details outlined on the form.

What you need to do

  1. Government department or public sector body lets you know if you need to complete the form.
  2. Complete the form or prepare your own documents.
  3. Send your completed form or prepared documents by mail or email. Or send it to the deputy minister of the department or public sector body requesting the consent.
